Introduction :
Failure Modes and Effects Analysis (FMEA) is a Systematic Approach used by industries worldwide to reduce risk in implementing new designs and processes. Objective of FMEA is to identify potential and known failure modes, evaluate the risk associate with each failure mode and take action to reduce the risk. Considering its preventive focus, FMEA is a mandatory requirement of ISO/TS16949. FMEA can be applied to designs, processes, services and systems to reduce risk of failure. Most world class companies use FMEA extensively to ensure “right–first-time” implementation of designs, processes, services and systems. Thus understanding and application of FMEA is essential not only for engineers but also other service providers to assure consistent performance to their customers.
